Addressing Population Health Issues

HAP has created a dashboard and maps as resources to help hospitals and their community partners to better understand the health needs and challenges across Pennsylvania. The metrics included on this dashboard include screening, prevention, chronic, and acute conditions that influence a person’s health and shape the health of our communities.

Many of these resources can be found through the population health dashboard on this page, and more tools are in the works. The detailed county mapping by prevalence shows the challenges on a local level.

UPMC is expanding workforce development efforts through its “Pathways to Work” program, focusing on creating career “on-ramps” into high-demand health care and skilled trade professions.
