Guidelines: Population Health Overview
Population health programs focus on more than sudden illness and complex diseases. That’s why hospitals are working to keep people healthy and out of the hospital as well as taking excellent care of them when they are sick or injured. Hospitals are focused on preventive health and wellness and providing the right care in the right place at the right time. They are reducing health disparities to improve equal access to services and increasing patient engagement in decision making. Hospitals also are working with partners to address social determinants of health, such as housing, hunger, and public safety. The goal is to improve people’s health and quality of life and create cost-effective care processes.
