Social determinants of health (SDOH)—such as inadequate health care access, below average wages, poor housing conditions, and food insecurity—play a significant role in health outcomes. Identifying and addressing social determinants of health can help mitigate disparities and improve outcomes.

As a follow up to HAP’s first Health Equity Action Day, held during October 2023, a virtual sharing and learning session will be held as a platform to engage with colleagues and to continue the conversation. The session will focus on lessons learned from the workshop, equity opportunities on the horizon, and a chance to learn from participants currently engaged in their journey to implement next steps and action plans developed at the workshop.
