Case Study: Grove City Medical Center 2019 HAP Achievement Award

The goal:  Reduce the incidence of diabetes in our community by helping people with prediabetes avoid the development of type 2 diabetes.

Grove City Medical Center partnered with the local YMCA to provide Y-USA's Diabetes Prevention Program. The program is based on lifestyle intervention, behavior modification, and education.

Certified lifestyle coaches from both organizations facilitated year-long education programs for groups of eight-to-ten participants, who pledged to work towards and achieve goals of losing a minimum of 7 percent of their body weight and engage in at least 150 minutes of physical activity weekly.

Results:  To date, 13 (about 40%) of the program’s 33 participants have achieved their goals. All participants have lost weight and either eliminated or reduced their need for one or more medications to control their cholesterol or blood pressure.

Most importantly, every participant has committed to taking the path to better health and away from type 2 diabetes.
