HAP Achievement Awards: Geisinger

Geisinger's award-winning teamHAP's annual Achievement Awards honor hospitals and health systems for their innovation, creativity, and commitment to patient care. The Achievement Awards showcase and share member hospitals’ and health systems’ successful programs and best practices.

Geisinger received the Community Champions Award for establishing a Social Needs Resource Hub that connects patients with essential resources and addresses unmet social needs.

Here’s what you need to know:

  • The challenge:  Unmet social needs, including food, housing, transportation, financial strain, and emotional wellness, created barriers to care and contributed to avoidable health disparities.
  • Addressing the Issue:  Reviewing the current process revealed fragmented workflows, limited follow-up capacity, low closed-loop referral rates, and overburdened clinical teams as key contributors.
  • Taking Action:  A centralized Social Needs Resource Hub streamlined navigation, strengthened community partnerships, integrated real-time referral tools and EMR documentation, and helped patients and members connect to essential resources more consistently.
  • Results:  In its first four months, the hub supported 1,245 patients and members, increased successful resource connections from less than 5% to 83%, achieved strong transportation approval rates, and helped resolve emotional wellness needs more quickly.
  • Advancing Care:  The team framed the hub as a way to bring social care more fully into the health care experience, noting that “The Social Needs Resource Hub (SNRH) demonstrates measurable, equitable, and sustainable outcomes while serving as a scalable, replicable model for health systems seeking to operationalize social care coordination at population scale.”
