Though Medicare is pushing for 90 percent of providers to participate in value-based care programs by 2030, only 2 percent of Medicare providers are participating in Chronic Care Management (CCM). CCM is a profoundly beneficial Medicare program for both practices and patients that is underutilized. It is a coordinated approach to health care that helps patients with multiple chronic conditions manage and improve their overall health and well-being. CCM has been shown to improve patient outcomes, increase patient satisfaction, reduce health care costs and provide a recurring stream of revenue.
