Decide for Yourself: A Guide to Advance Directives
HAP worked closely with the Pennsylvania Medical Society to produce this updated patient brochure in response to Pennsylvania’s recently enacted advance directives law, Act 169 of 2006. The new law was effective January 29, 2007, and has a significant impact on health care providers and patients. The brochure is in an easy-to-read question and answer format that informs patients about what advance directives are, how to make an advance directive, and what to consider in making one. This updated version incorporates information on health care power of attorney and health care representative, which are new provisions in Act 169 of 2006.

Suggested uses include keeping copies of the brochures in waiting rooms and in admission packets.

They can be ordered through HAP’s Membership Services Department in bulk quantities. This brochure can be customized. Please reference the specific order form for complete information.
