Comprehensive Care Transitions Help Reduce Readmissions for Heart Attack Patients

A study of heart attack patients at Thomas Jefferson University Hospital shows that consistent discharge education and hand-off to non-hospital physicians can decrease by 48 percent the likelihood of sudden, premature return to the hospital after discharge. The research was recently published in the American Journal of Medical Quality.

Wolf Administration Holds First Warm Hand-off Regional Summit

Yesterday, the Wolf Administration held the first of six Warm Hand-off Regional Summits in Harrisburg. With Pennsylvania facing the fourth-highest opioid death rate in the country, these summits are intended to promote collaboration among state government, county governments, county and regional services, private insurers, HAP, behavioral health facilities, and the hospital and provider communities.

DHS Creates New PASRR Website

The Pennsylvania Department of Human Services (DHS) has created a new website to assist hospitals in completing the Preadmission Screening and Resident Review (PASRR) for patients being discharged to a Medicaid-certified nursing facility. The PASRR process is a federal requirement to assess the patient’s needs to ensure the most appropriate setting.

CMS to Test Clinical-Community Collaboration to Assist Patients, Align Services

In an effort to support local communities in addressing the health-related social needs of Medicare and Medicaid beneficiaries, the Centers for Medicare & Medicaid Services (CMS) will provide support to 32 organizations—including Pennsylvania’s Reading Hospital—working to bridge the gap between clinical and community services.
