HAP has suspended all in-person education programs and member meetings through the end of April and will extend this if circumstances warrant. We want to keep you safe, support your COVID-19 response efforts and not pull you away from your focus on the health and safety of the Commonwealth. This conference will be canceled or postponed to later in the year. HAP staff will contact registered participants with further information and will be issuing full refunds for canceled programs.
The movement of patients between one care setting or care provider and another is fraught with opportunities for complications and breakdowns, which impact patients, their families, care providers, and ultimately the system as a whole. By improving care transitions, providers can engage patients, share care plans, and coordinate efforts to ensure that patients receive the right care at the right time, helping reduce unnecessary hospital readmissions.
HAP’s Care Transitions Workshop will feature national, Pennsylvania, and regional perspectives on innovative strategies to improve transitions in care, with a focus on participation, action, and collaboration. Speakers will share their insights and expertise on person-centered, effective practices and methods for overcoming barriers. A combination of plenary speakers, roundtable discussions and networking time will provide multiple opportunities for translating ideas into action.
