This safety readiness survey is the first in this series of initiatives that will help you reinforce a "culture of safety." HAP encourages you to use this self-assessment tool to determine your hospital's and health system's safety readiness.

Your personal involvement, along with your senior management team, in completing the survey will create additional opportunities for dialogue in your organization about internal safety readiness. You and your senior management team should complete the survey without seeking input from department managers, as this tool is designed to help you understand your organization's awareness and approach to the issue of patient safety.

This is a self-assessment tool. However, we would be very interested in hearing more about particular safety focuses and initiatives that evolve as a result of this assessment effort.
