HAP has revised its “A Statement of the Patient’s Rights & Responsibilities” brochure and poster to reflect the requirements of HIPAA which became effective April 14, 2003. Sample copies of both the brochure and poster (English and Spanish versions) can be downloaded below.

The brochure and poster can be ordered through HAP’s Membership Services Department in bulk quantities. This brochure can be customized. Please reference the specific order form for complete information.
