Contract and Letter of Commitment
2008 Joint Commission Continuous Service Readiness (CSR) Program

The Hospital & Healthsystem Association of Pennsylvania (HAP) has recently renegotiated a contract with Joint Commission Resources (JCR) to offer the Continuous Service Readiness (CSR) Program in Pennsylvania. The attached materials reflect the negotiated contract and associated fees for hospitals that elect to participate in the program. Hospitals and health systems that elect to participate in the Joint Commission Resource’s CSR program need to submit:
- A signed 2008 Continuous Service Readiness Letter of Commitment (Exhibit A).
- A signed Business Associate Agreement (Exhibit B or provide your own organizational BAA approved by JCR).
- A selected and signed CSR subscription package (Exhibit C).
The information can be submitted electronically or by mail to Linda Siegler, manager, professional services, CSR, Joint Commission Resources, no later than December 10, 2007.
