Patient Rights & Responsibilities Brochure and 12" x 18" Poster

REVISED TO REFLECT THE NEW CMS PATIENT VISITATION REGULATIONS WHICH ARE EFFECTIVE JANUARY 18, 2011.


HAP recently revised this brochure and poster to reflect new Centers for Medicare & Medicaid (CMS) Conditions of Participation regarding patient visitation.

HAP worked closely with members to produce this updated patient rights and responsibilities brochure and poster. Both pieces are in an easy-to-read format that informs patients about their rights related to: communication; informed decisions; advanced directives; visitation; care panning; care delivery; privacy and confidentiality; hospital bills; and complaints. The responsibilities section addresses: respect and consideration; safety; providing information; refusing care; charges; and cooperation.
 
This updated version addresses The Joint Commission’s patient-centered care standard on visitation and language and health literacy requirements. The brochure was reviewed by The Joint Commission and the Pennsylvania Department of Health before publication.
 
Suggested uses include keeping copies of the brochures in waiting rooms and in admission packets. Sample copies of the brochure and poster can be downloaded below. Permission must be obtained from HAP for any reprints of the brochure or its content.
